Appointments are essential for fittings, since Doctor Orders are necessary for Medicare and most insurances. Medicare orders older than 12 months will have to be renewed before the appointment.

Doctor Orders should contain the following information:
Date of Order
Patient name, address, phone #, and date of birth
Doctor's full name, address, phone #, fax #,  NPI #
Clinical information, such as diagnosis, ICD 9 or 10 Code, & type of surgery
Supplies ordered with amount of each such as:
Post-mastectomy camisole (L8015)                      QTY __ 
Mastectomy bras (L8000)                      QTY__
Silicone breast prosthesis  (L8030)  Lt RT or Bilateral    QTY__
Non-silicone breast prosthesis (L8020)  Lt Rt or Bilateral   QTY__
Signed and dated signature of the Doctor  (a stamped signature is not acceptable)

What to bring to your appointment:
Medicare card (if applicable) and/or current insurance cards.
Form of payment (Always A Woman accepts Master Card, Visa, Discover, most debit cards, and personal checks.)
